You will be notified in writing whether your request has been approved or denied. If approved, you will be notified of the access fee to be paid upon access. If denied, you will be given adequate reasons for the refusal and your appeal rights.

If you are prevented by a disability from reading, viewing, or listening to the record in the form of access provided, state your disability and indicate in which form the record is required.

G. Particulars of Right to be Exercised or Protected

If the provided space is inadequate, please continue on a separate folio and attach it to this form. The requester must sign all additional folios.

H. Fees

I. Declaration

I understand that:

• I must use the prescribed Form 2 for PAIA requests
• A request for access to a record containing personal information about myself will be processed without a request fee
• A request for access to a record, other than a record containing personal information about myself, will be processed only after a request fee has been paid
• Access fees may be charged for reproduction of the record
• If the request is granted, I will be notified of the amount of the access fee to be paid
• The access fee must be paid before any record will be disclosed
• I may lodge an internal appeal if my request is refused
